BENEFITS
HEALTH INSURANCE
Medical/Dental/Vision is available to all full-time employees of Central Refrigerated Service, Inc. and their eligible dependents beginning on the first day of the month following two full months of employment. Ex: Hired January 1 effective April 1.
After five (5) years of continuous, full-time employment, the Company pays 1/4 of the health insurance premiums. After ten (10) years of continuous, full-time employment, the Company pays 1/2 of the health insurance premiums. after fifteen (15) years of continuous, full-time employment, the Company pays 3/4 of the health insurance premiums.
CIGNA MEDICAL
$29.29 (HRA completed) or $43.70 (HRA not completed) per week for Single coverage (employee only)
$82.61 (HRA completed) $115.75 (HRA not completed) per week for Two Party coverage
$88.89 (HRA completed) or $137.88 (HRA not completed) per week for Family coverage.
CIGNA DENTAL

DISCOVERY BENEFITS FLEXIBLE SPENDING ACCOUNT
An FSA allows you to estimate your annual out-of-pocket health care and dependent care expenses and contribute that amount, via payroll deductions, to your account before taxes are calculated.
The FSA limit for healthcare expenses is $4,000 per plan year
The FSA limit for dependent childcare expenses is $5,000 per plan year (or $2,500 for married filing separately)

VACATION
After one (1) year of continuous full-time employment, full-time employees are eligible to receive paid vacation time.
1 year of active employment = 1 week pay
2 - 4 years of active employment = 2 weeks pay
5 years + of active employment = 3 weeks pay
401(K)
All employees of Central Refrigerated who are at least 21 years of age and who are not non-resident aliens or employees whose employment is covered by a collective bargaining agreement, are eligible to participate in the plan. If you are an eligible employee, you may elect to begin making salary reduction contributions to the plan after you have completed 90 days of service with Central Refrigerated. You are eligible to share in Central Refrigerated matching contributions after one year of service with Central Refrigerated.
You can elect to contribute between 1% and 75% of your pay on a pre-tax basis, not to exceed the annual dollar maximum imposed by the IRS.
Once you complete one year of service, you will share in Central's matching contributions. Central Refrigerated will make quarterly contributions to the plan, which will be allocated to participant accounts. The match will be based on your pre-tax contributions, up to 5% of your pay. The amount of Central Refrigerated's matching contributions will vary each quarter depending on the Company's operating ratio, in accordance with the following schedule:

Vesting means that the assets in your plan account become non-forfeitable. You are always 100% vested in your own contributions and their earnings.
